District Hazard and Risk Register

Purpose. This register records the operational, workplace and organisational risks identified across the district, their controls and the person responsible for each control.
1.How the register is used
The register is the single record of identified hazards and risks for the district. Every hazard report, incident review, audit finding and debrief that identifies a new risk must result in an entry or an update. The register is reviewed at every quarterly governance meeting and owned by the District Superintendent.
2.Rating risks
Each risk is rated for likelihood and consequence using the service's five by five matrix, before and after controls. Risks rated high or extreme after controls must have a treatment plan with a named owner and a due date. The rating must be reviewed whenever a related incident occurs.
3.Current operational risks
The register currently lists the following operational risks with their controls. Each entry names a control owner and the date of the last review.
- Assault on members at family violence incidents: two-member attendance, body worn camera, operational safety training
- Vehicle collisions during urgent duty driving: driver authorisation levels, pursuit controls, fatigue management
- Self-harm by persons in custody: intake assessment, welfare check intervals, cell design review
- Exposure to hazardous drugs during seizure: gloves, masks, field test kits, decontamination
- Fatigue on night shift rotations: maximum hours rule, rest breaks, supervisor checks
- Loss of exhibit continuity: exhibit procedure, dual control safes, quarterly audits
4.Current workplace risks
The register lists workplace hazards at each site, including manual handling in the exhibit rooms, slips in the vehicle wash bay, and psychological harm from exposure to traumatic incidents. Controls include mechanical aids, non-slip surfaces and the peer support and psychology programs. Site hazard inspections are conducted monthly.
5.Review and reporting
Control owners must confirm each quarter that their controls are in place and working. New risks must be added within five business days of identification. The register summary is reported to the regional command with the quarterly governance report.
